The Medical History page is comprised of three tabs - Personal, Family and Social. Each of these tabs can only be updated by the patient.
Select a link from the navigation menu on the left for more information.

The Personal History tab enables the patient to enter current member health information, recent medical tests and medical history. This tab displays as the default page. |

The Family History tab provides the ability to enter family members and their health history. |

The Social History tab enables the patient to enter information relating to their occupation, education, religion, alcohol/drugs, pets, stress level and travel/sleep. |
