The Personal History tab is the first to display. This tab provides the ability to update personal information, cancel the update, or print to a PDF report.
This page also calculates Body Mass Index (BMI) based on the height and weight entered by the patient. Additional BMI information can be viewed by clicking the
icon.
The Personal History tab is divided into three sections.
- Current Member Health Information
- Recent Medical Tests
- Medical History
The Current Member Health Information section provides the ability to add information relating to general health.
The Recent Medical Tests section provides the ability to add specific test information such as the type of test administered and the time period the test was performed.
The Medical History section provides the ability to add known disorders, diseases, pain or injuries. Update each section accordingly and click save to save the information or cancel to cancel the update.
A message displays stating the medical history successfully updated.
