The outpatient and office visit section displays the following based on current available data:

• Date of Service
• Diagnoses
• Provider Name
• Facility Name

To view detailed information click a visit.

The visit details may display additional information such as:

• Procedures
• Referring Provider

To collapse the visit details click close.

Additional capabilities are also available:

• View All (displays the last 12 months of outpatient/office visit information on the Visits page)
• Notes (ability to add a comment relating to the visit)

If no data exists, the PHR displays “No visits have been identified for this patient.”
