Patient information is displayed at the top of the Health Summary page, which includes:
- Name
- Address
- Date of Birth
- Gender
- Race
- Marital Status
- Emergency Contact
- Contact Relationship
- Contact Telephone
Most of this information can be updated in the PHR with the exception of name, address and date of birth. To update name or address information, call the customer service number located on the back of your health insurance identification card.
Additional information can be viewed by clicking “View More Patient Information.”
Patient Image
Images can be submitted to enhance the user’s PHR. All pictures must be submitted for review. Images that contain graphic content or nudity will not be approved.
Photo requirements include:
- Maximum image dimensions 120 x 120 pixels
- Maximum file size is four megabytes
- Only JPEG, GIF or PNG file types are accepted
Click the picture area to choose an image.
The Image Upload window displays. Click Choose File.
Choose a photo and click open.
Enter a current email address and click submit.
A message displays stating the image was successfully created. Click close.
An email will be sent confirming that the photo was received.
Once the photo is reviewed a second email will be sent noting the approval or rejection.
Upon approval, the photo will display on the Health Summary and Patient Profile pages.
