The Chronic Conditions / Risk Factors area provides alerts for possible chronic conditions as well as patient entered allergies.

This information is designed to assist providers in making informed decisions about a patient’s health care. Knowing a patient’s chronic conditions and risk factors are helpful, especially in emergency situations. Patient information, chronic conditions and risk factors display on each page of the PHR.

	The PHR tracks certain procedure and diagnosis codes relating to chronic conditions. If a patient has two or more office visits, an emergency room visit, or hospital admission with a chronic condition diagnosis in the past 24 months, the PHR will list the condition here. This information is obtained from claims data and cannot be entered or altered by the patient.
	Patient allergies are identified as risk factors. Allergies are entered by the patient on the Allergy page.
