The Provider List page displays the medical provider that has been identified as being primarily responsible for coordinating the patient’s health care. This health provider is also known as the Primary Care Physician or PCP.
Changes to the PCP can only be made through customer service. Contact information is located on the back of the health plan identification card. PHR staff cannot update PCP information.
The Provider List section displays the name of the insurance carrier and the identified PCP. The following information is included:
- Provider Name
- Specialty
- Address
- City
- State
- Zip
If a PCP is not identified, the provider list will show none selected.
