Users that log directly into the PHR are set up and maintained by the PHR staff. Q‐Chart access is only granted in certain circumstances as most users log in through My Blueprint. Verbal or written communication must be provided by the PHR staff to access the secure Q‐Chart website.

During this process, a user ID and activation code is provided. Once the user ID and activation code are received, the user is directed to the secure Q‐Chart website: https://secure.qchart.net. Enter the user ID and click log on to complete the registration process.
